EPIDEMIOLOGY

The Department of Epidemiology initiated with the objective of studying the distribution and determinants of diabetes and its complications in the population has conducted numerous studies including the Chennai Urban Population Study (CUPS), Chennai Urban Rural Epidemiology Study (CURES), Indian Council of Medical Research (ICMR)–India Diabetes (INDIAB) study, Prospective Urban and Rural Epidemiology (PURE) Study, Chunampet Rural Diabetes Prevention Project (CRDPP), Women in India with GDM Strategy (WINGS), Stratification of risk of Diabetes in Early pregnancy (STRiDE), Precision Cardiovascular Disease Phenotyping and Pathophysiological Pathways in the CARRS cohort (Precision-CARRS), etc. These studies have fostered collaborations and partnerships with prestigious universities and research organizations both within India and internationally. Such collaborations have enabled the sharing of expertise, resources, and technology, enhancing the quality and impact of ongoing diabetes research.

The Department of Epidemiology, headed by Dr. Deepa Niranjan, is dedicated to understanding the distribution, determinants, and impact of diabetes and its complications within diverse populations. Since its inception, the department has undertaken several landmark studies, including the Chennai Urban Population Study (CUPS), Chennai Urban Rural Epidemiology Study (CURES), ICMR–India Diabetes (INDIAB) Study, Prospective Urban and Rural Epidemiology (PURE) Study, Chunampet Rural Diabetes Prevention Project (CRDPP), Women in India with GDM Strategy (WINGS), Stratification of Risk of Diabetes in Early Pregnancy (STRiDE), and Precision-CARRS. Through these extensive research initiatives, the department continues to make significant contributions to public health, diabetes prevention, and evidence-based policymaking.

Projects

Facilities and technologies

MDRF houses a biobank at its Siruseri facility (29 kms from the heart of Chennai city, Tamilnadu). The 3,700-square-foot facility includes liquid nitrogen containers (–196°C) and freezers (–20°C or –80°C), with over 800,000 samples stored. All freezers are on backup emergency power and are monitored around the clock. The biobank at MDRF provides a mechanism to simplify and streamline the process of sample collection, retrieval and disbursement, thereby provide exceptional support to research focusing on diabetes and its complications. MDRF is supported by the information technology (IT) infrastructure, using barcode facility, for sample tracking and management, annotation and other methods of data collection, as required. The sequence of steps involved in the biorepository are as follows: initial process of collecting, processing (aliquoting & labelling), freezing, anonymizing, and storing the samples with its corresponding clinical information in a database. It adheres to ICMR guidelines and supports research on diabetes, providing insights into disease mechanisms and facilitating long-term studies.

Achievements

    • Largest epidemiological data on diabetes – ICMR-INDIAB study: Through this ICMR-INDIAB study, the largest epidemiological study done in India so far, we report for the first time, prevalence of diabetes and other metabolic disorders from a nationally representative sample from 31 states/union territories. This valuable information will assist the national and state Governments in formulating policies for improving the lives of people and diabetes care in their respective states.
    • Metabolic NCD burden: Urban-rural differences, state wise, region wise and national prevalence of all cardio-metabolic disease, such as, diabetes, prediabetes, obesity, hypertension, dyslipidemia and cardiovascular disease.
    • ABC Control across the nation: Achievement of diabetes treatment targets which remain suboptimal in the Asian Indian population with self-reported diabetes. These suggest that the urgent need for earlier and more effective intensification of treatment so as to comprehensively reduce cardiovascular risk in this population.
    • HbA1c – new targets & normative values: Recommendation for using age-specific cutoffs while utilizing HbA1c to diagnose diabetes and prediabetes, so as to minimize the risk of overdiagnosis and unnecessary initiation of treatment in elderly people who could have physiological increase in HbA1c levels.
    • Impaired kidney function – national level data: Impaired kidney function is a significant public health concern in India, particularly among those with diabetes and hypertension. Early identification and treatment of risk factors, regular screening for impaired kidney function in those at risk and prompt institution of evidence-based interventions could prevent undue burden on individuals, families and healthcare systems.
    • Importance of migration data: Observation that rural-to-urban migration is associated with increased risk of developing diabetes and other cardiometabolic abnormalities.
    • Physical activity levels: Physical inactivity in India revealed that fewer than 10% engage in recreational physical activity, highlighting the urgent need to promote active lifestyles to curb the growing epidemics of diabetes and obesity.
    • Knowledge and awareness of diabetes: Knowledge and awareness of diabetes in India and reported that it is poor in rural areas, underscoring the need for conducting large scale diabetes awareness and education programs.
    • Regional prevalence of GDM (early GDM vs. late GDM): Our findings highlight the high prevalence of GDM among Indians, underscoring the need for monitoring for GDM throughout pregnancy, starting from the first trimester. By enhancing awareness, promoting timely diagnosis, and offering proper care, the risks associated with GDM can be effectively reduced, leading to better maternal and neonatal health outcomes.
    • Indian Diabetes Risk Score (IDRS): The Indian Diabetes Risk Score (IDRS) was developed by MDRF as a simple, cost-effective, and non-invasive screening tool to identify undiagnosed type 2 diabetes in the Indian population using four easily measurable parameters (age, family history of diabetes, physical activity & waist circumference). IDRS was a significant step in diabetes prevention efforts in India, providing a quick and cost-effective screening for diabetes in Asian Indians. Performance of MDRF-IDRS is evaluated across the nation and is found to be suitable, for cost effective screening for diabetes in Asian Indians.
    • Composite risk score for GDM: The STRiDE study showed that early pregnancy HbA1c, on its own or as a composite risk score with common risk factors, can be used to predict about 50% of GDM at 24–28 weeks of gestation. The need for OGTTs can be reduced by 50–64% in different populations by using venous or point-of-care HbA1c. These findings can help to improve overall screening rates for GDM in LMICs and reduce the burden of OGTTs. This can help to redirect the resources required for conducting OGTTs to earlier intervention for women at higher risk. For effective risk stratification, population-specific thresholds are needed.
    • GDM screening and model of care: The WINGS (Women in India with GDM Strategy) study is a significant initiative focused on GDM in India, that developed and evaluated a comprehensive model of care (MOC) tailored for women with GDM in low- and middle-income countries. The WINGS study addresses critical gaps in screening, diagnosis, management, and follow-up for women with GDM, especially in resource-constrained settings.
    • Diabetes prevention in rural areas: The Chunampet Rural Diabetes Prevention Project (CRDPP) is a significant initiative aimed at addressing the diabetes epidemic in rural India, which implemented wide-scale screening for diabetes and prediabetes along with screening for diabetic complications such as retinopathy, neuropathy, kidney, foot, and heart complications using telemedicine technologies.
