EPIDEMIOLOGY

The Department of Epidemiology initiated with the objective of studying the distribution and determinants of diabetes and its complications in the population has conducted numerous studies including the Chennai Urban Population Study (CUPS), Chennai Urban Rural Epidemiology Study (CURES), Indian Council of Medical Research (ICMR)–India Diabetes (INDIAB) study, Prospective Urban and Rural Epidemiology (PURE) Study, Chunampet Rural Diabetes Prevention Project (CRDPP), Women in India with GDM Strategy (WINGS), Stratification of risk of Diabetes in Early pregnancy (STRiDE), Precision Cardiovascular Disease Phenotyping and Pathophysiological Pathways in the CARRS cohort (Precision-CARRS), etc. These studies have fostered collaborations and partnerships with prestigious universities and research organizations both within India and internationally. Such collaborations have enabled the sharing of expertise, resources, and technology, enhancing the quality and impact of ongoing diabetes research.

To provide solid evidence-based knowledge on non-communicable diseases (NCDs) in general and diabetes in specific, its determinants and methods of control in the population that would provide a basis for health action.

The Department of Epidemiology, headed by Dr. Deepa Niranjan, is dedicated to understanding the distribution, determinants, and impact of diabetes and its complications within diverse populations. Since its inception, the department has undertaken several landmark studies, including the Chennai Urban Population Study (CUPS), Chennai Urban Rural Epidemiology Study (CURES), ICMR–India Diabetes (INDIAB) Study, Prospective Urban and Rural Epidemiology (PURE) Study, Chunampet Rural Diabetes Prevention Project (CRDPP), Women in India with GDM Strategy (WINGS), Stratification of Risk of Diabetes in Early Pregnancy (STRiDE), and Precision-CARRS. Through these extensive research initiatives, the department continues to make significant contributions to public health, diabetes prevention, and evidence-based policymaking.

Projects

Indian Council of Medical Research (ICMR)–India Diabetes (INDIAB) study [2008–2020]

The ICMR-INDIAB study is the largest epidemiological studies on diabetes in the country with over 120,000 people covering every state of India. It is a cross-sectional, population-based survey of adults aged ³20 years. The study sampled 33,537 urban and 79,506 rural residents (overall n=113,043) in 31 states/ Union Territories (UT) of the country using a stratified multistage sampling design. In order to obtain a truly representative sample of the population, we used three-level stratification based on geography, population size, and socioeconomic status of each state. Using a systematic sampling method, 24 and 56 households were selected from urban and rural areas by door-to-door survey. The study was conducted in a phased manner between November, 2008, and December, 2020. In phase 1, four regions representing the south (Tamil Nadu), north (Chandigarh), east (Jharkhand), and west (Maharashtra) of the country were studied from Oct 18, 2008, to April 16, 2010. The remaining states were surveyed as follows: phase 2 consisted of undivided Andhra Pradesh (subsequently divided into Andhra Pradesh and Telangana), Bihar, Gujarat, Karnataka, and Punjab (survey period, Sept 24, 2012, to July 26, 2013), the north east phase included Assam, Arunachal Pradesh, Manipur, Meghalaya, Mizoram, Nagaland, Sikkim, and Tripura (survey period, Dec 5, 2011, to Nov 14, 2017), phase 3 included Delhi, Madhya Pradesh, Rajasthan, and Uttar Pradesh (survey period, Aug 24, 2017, to March 8, 2018), phase 4 included Kerala, Goa, Puducherry, Haryana, and Chhattisgarh (survey period, Dec 10, 2018, to July 10, 2019), and phase 5 included Himachal Pradesh, Uttarakhand, Odisha, and West Bengal (survey period, Sept 1, 2019 to Dec 31, 2020).

The ICMR–INDIAB study provides accurate and comprehensive data on all metabolic NCDs in India. It provides valuable information on the distribution of risk factors in the regions studied. This study is also unique in that it is designed to cover both rural and urban areas and provide estimates for prediabetes, dyslipidemia, hypertension, obesity, and the level of glycemic control among the confirmed cases of diabetes. The study helps to throw light on the health burden due to diabetes in India and also to plan measures for both control and prevention of diabetes.

ICMR-INDIAB study (follow-up study–Tamil Nadu & Arunachal Pradesh) [2023–2024]

The follow-up study of one state from the ICMR-INDIAB Phase I (Tamil Nadu) and one from the Northeast (Arunachal Pradesh) is recently completed, which aims to estimate the changes in prevalence of diabetes and prediabetes/ incidence risk in urban and rural areas of two selected regions of India and study the conversion rates to prediabetes and diabetes in those with normal glucose tolerance (NGT) and to diabetes in those with prediabetes. Two states, one in the mainland of India (with high prevalence of diabetes) and one in the north east (with low prevalence of diabetes), which were done nearly a decade ago, were selected to assess the increase in the prevalence rates over a 10-year period. This study also follows up those with diabetes to get details on their health status, quality of life, complication and mortality rates. The study also aims to assess the current prevalence of diabetes and prediabetes in urban and rural areas of the two selected states.

Two study designs were employed in this project:

(i) New cross-sectional door-to-door survey (to obtain the current estimates in the prevalence of diabetes and prediabetes in the selected two states) with a sample size of 4,000 individuals for each state, totaling to 8,000 individuals.              

(ii) Prospective follow up survey of individuals already studied previously in the selected two states – Tamil Nadu and Arunachal Pradesh (to determine the incidence risk diabetes and prediabetes and complications rates in those with diabetes and mortality rates). A total of 7,488 individuals who had given capillary blood samples for OGTT and fasting capillary blood samples (known diabetes) at baseline were included in the follow up study.

In Tamil Nadu, all the 104 PSU’s were completed (51 urban PSU and 53 rural PSU in both cross-sectional and prospective study). In Arunachal Pradesh, 79 PSUs (26 urban PSU and 53 rural PSU) in the cross-sectional study and 74 PSUs (21 urban PSU and 53 rural PSU) in the prospective study were completed.

ICMR-INDIAB study (Union Territories) [2023–2024]

The ICMR-INDIAB Union Territories study include five UTs, namely, Kashmir & Jammu, Ladakh, Andaman and Nicobar Islands, Lakshadweep and Dadra and Nagar Haveli and Daman and Diu. This study surveyed 8,198 individuals from five Union territories of India that includes three islands. Each UT has an urban component and a rural component (villages). This study not only assesses the burden of diabetes and prediabetes in UTs of India, but also obtains information on other cardio-metabolic risk factors. The primary objective of this study is to determine the prevalence of diabetes mellitus & prediabetes [Impaired fasting glucose (IFG) / Impaired glucose tolerance (IGT)] in the Union territories (UTs) of India namely Andaman and Nicobar Islands, Dadra and Nagar Haveli and Daman and Diu, Kashmir & Jammu, Ladakh and Lakshadweep. In all the five Union Territories, 8,198 participants were selected for the study, out of these 8,037 [98%] individuals participated in the study. Participation rates exceeded 97% across all surveyed regions: 97.8% in Jammu, 98.0% in Kashmir and Ladakh, 97.1% in the Andaman and Nicobar Islands, 98.2% in Lakshadweep, and 98.6% in Dadra and Nagar Haveli and Daman and Diu.

Ethnic specific risk stratification in early pregnancy for identifying mothers at risk of gestational diabetes mellitus in India [2016–2019]
STratification of Risk of Diabetes in Early pregnancy (STRiDE), is a longitudinal observational study that recruited pregnant women in early pregnancy and prospectively followed them until delivery to derive risk score for gestational diabetes mellitus (GDM), conducted between 2016 and 2019. The project developed a composite risk score that predicts GDM in early pregnancy using the conventional risk factors such as age, BMI and family history of type 2 diabetes along with an early pregnancy HbA1c. Leveraging the infrastructure and networking facility at MDRF (and its associated clinical centres), first-time in the country, a large cohort of pregnant women (>3,000) were recruited for the project as early as in 16 weeks of gestation with a follow-up until delivery. The study showed that early pregnancy HbA1c, on its own or as a composite risk score with common risk factors, can be used to predict about 50% of gestational diabetes at 24–28 weeks of gestation. The risk ratios of developing GDM were different in different populations, highlighting the need for population-specific thresholds. Using two thresholds for rule-in and rule-out gestational diabetes helps with risk stratification in early pregnancy: those at highest risk (rule-in gestational diabetes), lowest risk (rule-out gestational diabetes) and medium risk (requires an OGTT at 24–28 weeks). The need for OGTTs can be reduced by 50–64% in different populations by using venous or point-of-care HbA1c. These findings can help to improve overall screening rates for gestational diabetes in LMICs and reduce the burden of OGTTs. This can help to redirect the resources required for conducting OGTTs to earlier intervention for women at higher risk. For effective risk stratification, population-specific thresholds are needed. Future randomized controlled trials are required to investigate whether this approach helps to reduce adverse pregnancy outcomes or reduce adverse offspring programming.

Prospective Urban Rural Epidemiology Study (PURE) [Initiated in 2002]

The PURE (Prospective Urban Rural Epidemiology) study, a large-scale cohort initiative led in collaboration with McMaster University, Canada, involves the MDRF and focuses on tracking changes in lifestyle, risk factor development, and chronic disease patterns across urban and rural regions worldwide. This ongoing study aims to understand how modernization, urbanization, and globalization affect health behaviors and risk factors, influencing a range of chronic conditions such as cardiovascular disease, diabetes, lung diseases, cancers, kidney disease, brain health, and injuries. With a global scale encompassing 225,000 participants from over 1,000 communities in 27 high-, middle-, and low-income countries, the PURE study reflects a variety of socioeconomic and cultural backgrounds. Participating countries include Argentina, Brazil, Canada, China, Colombia, India, Kenya, Kuwait, Russia, Thailand, the UAE, Zimbabwe, and more, offering a comprehensive view of health across diverse regions.

MDRF, as one of the five sites in India, is part of the PURE cohort and the baseline number includes a total of 5,241 participants, 2,741 from urban areas and 2,500 from rural areas and are being followed up prospectively. The PURE study provides important and novel information on the causes of the major health burden, due to obesity, diabetes, and CVD globally. Its size, scope (both societal and individual influences) and inclusion of individuals from a diverse group of countries, enables it to discern truly universal risk factors. It provides valuable insights regarding interactions between societal, household, individual, and genetic determinants of the disease, which will influence CVD and other chronic disease prevention in India and globally.

Precision Cardiovascular Disease Phenotyping and Pathophysiological Pathways in the CARRS cohort (Precision-CARRS) [2022–2027]

The overarching goal of the proposed Precision Cardiovascular Disease Phenotyping and Pathophysiological Pathways in the CARRS cohort (Precision-CARRS) Program Project Grant (PPG) is to transform the current cardiovascular disease (CVD) risk prediction paradigm that is based on conventional, imprecise atherosclerotic cardiovascular disease (ASCVD) risk factors to personalized and precise disease prediction and early-stage detection to promote cardiovascular health. The PPG is funded by the National Institute of Health (NIH), US. The PPG supports three complementary cores: (1) Administrative and Field Coordination (AFC); (2) CVD Phenotyping; and (3) Data Management and Analytics (DMA), and four interlinked projects built within the Precision-CARRS. All projects leveraged existing data and incorporated new data collection, with common endpoints focused on subclinical and clinical cardiovascular disease (CVD). The objectives of the four inter-linked projects are to:
Project 1: Estimate the prevalence, incidence, and predictors of subclinical and clinical vascular and myocardial disease, with exploration of pathophysiologic pathways.
Project 2: Investigate the role of environmental exposures (air pollution/built environment) on the development of subclinical and clinical vascular and myocardial disease.
Project 3:  Investigate the multi-omics determinants of subclinical and clinical vascular and myocardial disease.
Project 4- Early-Stage Investigator (ESI) Project:  Investigate spousal influences on subclinical and clinical vascular and myocardial disease.
CARRS recruited 21,864 people ages ≥20 years (19,275 gave bloods) that are representative of adults residing in Delhi and Chennai, India, in two waves using identical methods (CARRS-1 assembled in 2010-11, n=12,271; 10,205 with bio-specimens; and CARRS-2 assembled in 2015-16, n=9,593; 9,070 with bio-specimens). Participants from both CARRS-1 and CARRS-2 are being prospectively followed as part of Precision-CARRS. The cardiovascular imaging assessments included in the project are electrocardiogram (ECG), echocardiogram, coronary computed tomography (CT), carotid intima-media thickness (CIMT), and arterial stiffness measurements.

Precision–CARRS–Brain: Multi-domain (genetic, socio-behavioural, vascular) risk factors and prediction of Alzheimer’s Disease continuum in South Asians in India

The Precision-CARRS-Brain study is proposed to address the growing burden of Alzheimer’s disease and related dementias (AD/ADRD) in South Asians, who face heightened genetic, vascular, and socio-behavioral risks but remain understudied. Building on the robust Precision-CARRS (P-CARRS) cohort in Delhi and Chennai, which has accrued 15 years of detailed socio-demographic, behavioral, environmental, and cardiometabolic data, the study will integrate advanced AD phenotyping protocols aligned with National Institute of Aging (NIA) funded centers. Led by an interdisciplinary team of multiple PIs from Emory University, Madras Diabetes Research Foundation (MDRF), Centre for Chronic Disease Control (CCDC), All India Institute of Medical Sciences (AIIMS), New Delhi and National Institute of Mental Health and Neuro Sciences (NIMHANS) experts in cardiometabolic diseases, neurology, dementia, and diabetes. This project leverages cores in field coordination, data management, genetics, imaging, and neuropsychology. Over 8,000 adults aged ≥40 years will undergo multimodal assessments, including digital cognitive testing, plasma biomarkers (pTau217, NfL, GFAP), neuroimaging, retinal imaging, and whole genome sequencing. The aims are to (i) perform multimodal AD/ADRD phenotyping; (ii) investigate modifiable risk factors (socio-behavioral, environmental, and vascular/metabolic factors) and novel non-invasive biomarkers associated with the AD/ADRD continuum; and (iii) develop and validate a longitudinal machine learning prediction framework for AD/ADRD. This collaborative effort between US and Indian institutions will advance cross-cultural dementia research, provide unique insights into disease mechanisms in South Asians, and contribute to precision prevention and therapeutic strategies worldwide.

Drug Intervention for Prevention of type 2 Diabetes – (DIVINE) - A Randomized Control Trial [2025–2028]

The project titled “Drug intervention for prevention of type 2 diabetes among women with prior gestational diabetes mellitus and pre-diabetes in South Asia (DIVINE): A Randomized Control Trial” is being funded by Indian Council of Medical Research (ICMR).
The primary objective of the study is to evaluate the efficacy of combination of metformin and vildagliptin in prevention of diabetes in women with prior GDM and having prediabetes in the postpartum period. The target population includes women who experienced GDM in a recent pregnancy and who are currently diagnosed with prediabetes.
Study Sites: The trial is being conducted across 10 sites in India, representing diverse regions:

The recruitment target is 1,110 participants (approximately 111 women per site). Eligible participants must have a history of GDM within the past five years, and a current diagnosis of prediabetes, confirmed through an oral glucose tolerance test (OGTT). Screening is currently in progress.

Clinical Trial of use of Continuous Glucose Monitoring in pregnant women (CGM-GDM) [2025–2026]

The main objective of the study is to evaluate the utility of CGM metrics in diagnosing and monitoring eGDM (Early Gestational Diabetes Mellitus) and LGDM (Late Gestational Diabetes Mellitus) and their association with maternal and neonatal outcomes. The study plans to study 200 women in the first trimester of pregnancy i.e., before 14 weeks of pregnancy recruited from antenatal clinics in Chennai in whom a fasting plasma glucose and HbA1c test will be done. In all 200 pregnant women, CGM (Freestyle Libre) will be deployed. These women will be followed throughout the pregnancy and will receive CGM once again in the second trimester.

Facilities and technologies

MDRF houses a biobank at its Siruseri facility (29 kms from the heart of Chennai city, Tamilnadu). The 3,700-square-foot facility includes liquid nitrogen containers (–196°C) and freezers (–20°C or –80°C), with over 800,000 samples stored. All freezers are on backup emergency power and are monitored around the clock. The biobank at MDRF provides a mechanism to simplify and streamline the process of sample collection, retrieval and disbursement, thereby provide exceptional support to research focusing on diabetes and its complications. MDRF is supported by the information technology (IT) infrastructure, using barcode facility, for sample tracking and management, annotation and other methods of data collection, as required. The sequence of steps involved in the biorepository are as follows: initial process of collecting, processing (aliquoting & labelling), freezing, anonymizing, and storing the samples with its corresponding clinical information in a database. It adheres to ICMR guidelines and supports research on diabetes, providing insights into disease mechanisms and facilitating long-term studies.

Achievements

    • Largest epidemiological data on diabetes – ICMR-INDIAB study: Through this ICMR-INDIAB study, the largest epidemiological study done in India so far, we report for the first time, prevalence of diabetes and other metabolic disorders from a nationally representative sample from 31 states/union territories. This valuable information will assist the national and state Governments in formulating policies for improving the lives of people and diabetes care in their respective states.
    • Metabolic NCD burden: Urban-rural differences, state wise, region wise and national prevalence of all cardio-metabolic disease, such as, diabetes, prediabetes, obesity, hypertension, dyslipidemia and cardiovascular disease.
    • ABC Control across the nation: Achievement of diabetes treatment targets which remain suboptimal in the Asian Indian population with self-reported diabetes. These suggest that the urgent need for earlier and more effective intensification of treatment so as to comprehensively reduce cardiovascular risk in this population.
    • HbA1c – new targets & normative values: Recommendation for using age-specific cutoffs while utilizing HbA1c to diagnose diabetes and prediabetes, so as to minimize the risk of overdiagnosis and unnecessary initiation of treatment in elderly people who could have physiological increase in HbA1c levels.
    • Impaired kidney function – national level data: Impaired kidney function is a significant public health concern in India, particularly among those with diabetes and hypertension. Early identification and treatment of risk factors, regular screening for impaired kidney function in those at risk and prompt institution of evidence-based interventions could prevent undue burden on individuals, families and healthcare systems.
    • Importance of migration data: Observation that rural-to-urban migration is associated with increased risk of developing diabetes and other cardiometabolic abnormalities.
    • Physical activity levels: Physical inactivity in India revealed that fewer than 10% engage in recreational physical activity, highlighting the urgent need to promote active lifestyles to curb the growing epidemics of diabetes and obesity.
    • Knowledge and awareness of diabetes: Knowledge and awareness of diabetes in India and reported that it is poor in rural areas, underscoring the need for conducting large scale diabetes awareness and education programs.
    • Regional prevalence of GDM (early GDM vs. late GDM): Our findings highlight the high prevalence of GDM among Indians, underscoring the need for monitoring for GDM throughout pregnancy, starting from the first trimester. By enhancing awareness, promoting timely diagnosis, and offering proper care, the risks associated with GDM can be effectively reduced, leading to better maternal and neonatal health outcomes.
    • Indian Diabetes Risk Score (IDRS): The Indian Diabetes Risk Score (IDRS) was developed by MDRF as a simple, cost-effective, and non-invasive screening tool to identify undiagnosed type 2 diabetes in the Indian population using four easily measurable parameters (age, family history of diabetes, physical activity & waist circumference). IDRS was a significant step in diabetes prevention efforts in India, providing a quick and cost-effective screening for diabetes in Asian Indians. Performance of MDRF-IDRS is evaluated across the nation and is found to be suitable, for cost effective screening for diabetes in Asian Indians.
    • Composite risk score for GDM: The STRiDE study showed that early pregnancy HbA1c, on its own or as a composite risk score with common risk factors, can be used to predict about 50% of GDM at 24–28 weeks of gestation. The need for OGTTs can be reduced by 50–64% in different populations by using venous or point-of-care HbA1c. These findings can help to improve overall screening rates for GDM in LMICs and reduce the burden of OGTTs.     • GDM screening and model of care: The WINGS (Women in India with GDM Strategy) study is a significant initiative focused on GDM in India, that developed and evaluated a comprehensive model of care (MOC) tailored for women with GDM in low- and middle-income countries. The WINGS study addresses critical gaps in screening, diagnosis, management, and follow-up for women with GDM, especially in resource-constrained settings.
    • Diabetes prevention in rural areas: The Chunampet Rural Diabetes Prevention Project (CRDPP) is a significant initiative aimed at addressing the diabetes epidemic in rural India, which implemented wide-scale screening for diabetes and prediabetes along with screening for diabetic complications such as retinopathy, neuropathy, kidney, foot, and heart complications using telemedicine technologies.
